MTM PHARMACY TECHNICIAN CERTIFICATION SOFTWARE
* Reconcile medication and medical diagnoses between DCI electronic health record and MTM software application * Collect, organize, and document patient medication(s), medical diagnose(s), prescription insurance information and provider response(s) in electronic health record or MTM software system * Upload pharmacy related documents into electronic health records as needed * Facilitate person-to-person telephonic and video-conference based consultations for patients enrolled in MTM program ** Under the direct supervision of a Pharmacist, the Reach MTM Pharmacy Technician's responsibilities include: **Essential Duties and Responsibilities:** The overall goal is to work collaboratively as a member of the healthcare team to optimize patient care and improve overall outcomes. The MTM Pharmacy Technician will play an integral role in the provision of medication therapy management services and transitions of care working directly with the Pharmacist and other healthcare disciplines.
We are currently searching for a Reach MTM Pharmacy Technician to work in a progressive dialysis environment where Pharmacists and Pharmacy Technicians provide critical clinical services to patients with chronic kidney disease.
